We make a great effort in discovering all aspects relating to your presenting problem. Your practitioner will listen carefully to the history of your condition. They will work together with you to determine an appropriate customised treatment plan based on your physical needs and treatment goals.
Before your first appointment, our friendly Admin team will direct you to convenient online forms so that we can obtain some information about your medical history. We assure you of our confidentiality and privacy and will seek your informed consent prior to being assessed and treated.
In your initial appointment, you will be asked a variety of questions to help your practitioner gain an insight into your activities of daily living, function and overall general health. The information you give assists the practitioner in generating a working diagnosis and an appropriate management/treatment plan. We endeavour to treat all clients with the highest levels of empathy, respect, and dignity.
It is likely that your practitioner will also conduct a physical assessment. During this assessment, it may be required for your practitioner to make physical contact with you. Throughout the assessment, you are in control. Please let your practitioner know if you have any concerns with respect to the assessment process at any time.
Physiotherapy, exercise, chiropractic, MSK medicine, and massage therapy are considered safe and effective forms of treatment. However, like most interventions along with the sought benefits, there are possible side effects and responses to treatment that are unique to every individual. Your practitioner will provide you with information about the recommended treatment along with associated risks and benefits. Our practitioners are very skilled and will be able to offer you a variety of treatments to help you find outcomes you are comfortable with.
The risk related to certain treatments can increase if your health practitioner is not aware of certain health conditions. Please inform your health practitioner if you have:
- a pacemaker or heart condition
- suffer from blood clots, thrombosis or stroke
- suffer from diabetes
- are taking any medication.
New Patient Information
All new clients to Smart Health must complete a Patient Contact Details Form and a Medical History & General Health Form. For clients under the age of 16 years, we require an age specific Medical Health Form to be completed. Our forms are designed to be submitted online for your convenience. Smart Health treats all information submitted confidentially and securely in accordance with our Privacy Policy.
If you do not wish to submit forms online simply contact our Administration Team on 8293 1100 and we will ensure that you can complete any forms and questionnaires required for your appointment through other means.
To provide the highest standard of treatment and care, we also encourage you to complete one or more of our Questionnaires that best relate to the reason for your visit for review by our practitioners before your appointment.
